Included in the zip file are two Word documents and this read me file. There is one Word 2000 template and one Word 97 template. To use either of the two documents just double click on template that goes with the version of Word you use. The only important thing to remember when using the template is to use the Save As command the first time you save a new doc. An example of that would be to Save As (your child's name.doc. This prevents changes in the template. One tip you may find useful is to use your tab key instead of using the mouse to move from one cell to the next. Make a document for each child you have and then print several copies to have on hand should you ever have the need for it. I suggest updating the document's every year. If you don't have Microsoft Word you can download the Word Viewer from www.microsoft,com This will allow you to open the document and print the forms so that you can fill them in by hand.
